Ziel: Das Bestrahlungsplanungssystem (auf Englisch: TPS) erlaubt Konturierung der Zielgebiete, Simulation, Strahlkonfiguration, dosimetrische Berechnung und Planauswertung. Nachdem eine CT Aufnahme des Patienten zur Verfügung gestellt und die Planungsziele definiert sind, werden optimale Strahlungswinkel, Dosisleistung und Form des Multi-Leaf-Kollimators berechnet. Um korrekte Dosisberechnungen zu erzielen, muss das Strahlenmodell des TPS validiert werden. Methoden: Das TPS enthält gemessene Tiefendosiskurven, Querprofile und Dosisleistungsfaktoren des Linearbeschleunigers für jede Strahlungsenergie. Basierend auf Testmessungen mit simplen Feldern und komplexen Bestrahlungsplänen werden Strahlenmodellparameter justiert, bis ein optimales Strahlenmodell gefunden wird: - Dreißig Bestrahlungspläne unterschiedlicher Komplexität werden mithilfe der Modalität für intensitätsmodulierte volumetrische Rotationsbestrahlung (auf Englisch: VMAT) im TPS RayStation (RaySearch, Stockholm, Schweden) für reale Patienten erstellt. Diese Bestrahlungspläne werden auf die Phantomgeometrie umgerechnet. - Die Messungen werden mit dem Delta4 Phantom (ScandiDos, Uppsala, Schweden) ausgeführt. - Alle gemessenen Bestrahlungspläne werden anhand der Gamma-Index-Methode ausgewertet. - Basierend auf Messungen wird das Strahlenmodell für VMAT Behandlungen adaptiert. Resultate: Um bessere Patienten-spezifische Übereinstimmung zwischen berechneter und gemessener Dosis zu erhalten werden vier Strahlenmodellparameter geändert. Unter Verwendung des adaptierten Strahlenmodells für VMAT Behandlungen werden die Ergebnisse der Messungen verwendet, um die Richtigkeit der Dosisberechnungsalgorithmen in RayStation zu beurteilen. Rein rechnerisch erzielt RayStation mindestens so gute Dosisverteilungen wie das vorhergehende TPS Monaco (Elekta, Stockholm, Schweden). Zwischen den beiden TPS wird kein statistisch signifikanter Unterschied bezüglich der Planungsziele festgestellt, jedoch gibt es Hinweise auf eine bessere Schonung der kontralateralen Ohrspeicheldrüse bei Kopf- und Halstumoren durch RayStation. Schlussfolgerung: RayStation rechnet schneller, ist benutzerfreundlicher und bietet mehrere zusätzliche Software Tools an, und erzielt mindestens so gute Dosisverteilungen wie Monaco.

Purpose: The treatment planning system (TPS) allows contouring on images, simulation, beam configuration, dosimetric calculation and plan evaluation. After providing a CT scan of the patient and defining planning aims, the optimizer determines the optimal beam angles, shape of the multi-leaf collimator and dose rate. For accurate dose calculations, the beam model of the TPS needs to be validated. Methods: The TPS contains measured depth dose curves, off-axis profiles and output factors of the linear accelerator for each beam energy. By tweaking beam model parameters based on test measurements with simple fields and complex treatment plans, an optimal beam model can be found: - Thirty treatment plans with different levels of complexity are developed using the volumetric-modulated arc therapy (VMAT) modality in the TPS RayStation (RaySearch, Stockholm, Sweden) for real patients. These treatment plans are recalculated for the phantom geometry. - The measurements are performed using the Delta4 (ScandiDos, Uppsala, Sweden) phantom. - All measured treatment plans are evaluated using gamma index analysis. - Based on measurements, the beam model is adapted for VMAT treatment. Results: Four beam model parameters are changed for better patient-specific agreement between calculated and measured dose. Using the adapted beam model for VMAT treatment plans, the results of measurements are used to assess the dose calculation accuracy of the dose calculation algorithms in RayStation. Computationally, RayStation achieves at least as good dose distributions as the previous TPS Monaco (Elekta, Stockholm, Sweden). No statistically significant differences regarding clinical goals were found between the two TPS, but an indication for better sparing of the contralateral parotid gland for head-and-neck cases was found for RayStation. Conclusion: RayStation not only computes faster, is more user-friendly and offers multiple additional software tools, but also achieves at least as good dose distributions as Monaco.
