Motivation: Eine Resektion von Tumoren in der Brustwand führt oftmals zu einem Verlust der Stabilität des Brustkorbes wodurch eine Brustkorbrekonstruktion erforderlich ist. Bei großen Defekten kommen Titanplatten zum Einsatz, die bei 84.6% der Patienten aufgrund eines Implantatbruchs eine chirurgische Revision erfordert. Die Konstruktion der Rippenimplantate sowie deren Fixierung stellt eine große Herausforderung dar und muss für jeden Patienten individuell gestaltet werden. Die Kenntnis über die biomechanische Belastung einer Rippe wäre ein wesentlicher Fortschritt bei der Entwicklung von Rippenimplantaten. In dieser Masterarbeit wird daher ein FE-Modell des Brustkorbs erstellt, um die Rippenkinematik zu simulieren und die Spannungen innerhalb der Rippen auszuwerten. Material und Methoden: Um die Biomechanik des Brustkorbes mittels FEM korrekt abbilden zu können, wurde die Steifigkeit und das Deformationsverhalten von dem gesamten Brustkorb, den Rippenknorpeln und den Rippenwirbelgelenken an drei frischen Leichen an der Medizinischen Universität Graz gemessen. Für das FE-Modell wurde die Geometrie des Brustkorbes anhand von CT Daten abgeleitet und die Materialparameter wurden aus den Messungen übernommen. Kleinere FE Test Modelle von der Bandscheibe, den Rippenwirbelgelenken sowie dem Rippenknorpel wurden erstellt, um das Modellverhalten mit den Daten aus den Messungen und aus der Literatur zu validieren. Ergebnisse: Die Biegemessungen der Rippenknorpel haben ergeben, dass der mediane E-Modul in kranialer/kaudaler Richtung (E = 23.4MPa) größer ist als in ventraler/dorsaler Richtung (E = 15.3MPa). Der Schubmodul der Proben ist in ventral-kaudaler und dorsal-kaudaler Richtung identisch. Ein medianer Schubmodul von 9.4MPa wurde gemessen. Das Deformationsverhalten des Knorpels konnte mit dem FE-Modell nicht korrekt abgebildet werden. Der Fehler zwischen den simulierten und gemessenen Deformationen in kranialer Richtung beträgt 42%. Bei den Rippenwirbelgelenken wurde ein gemittelter ROM in der neutralen Zone von 15.5! ± 5.8! gemessen. Die Zugsteifigkeit der Gelenke ist in longitudinal, sagittal und transversal Richtung identisch. Das Deformationsverhalten der Rippenwirbelgelenke konnte mit dem FE-Modell gut abgebildet werden. Das Deformationsverhaltendes gesamten Thorax wurde mit und ohne Interkostalmuskulatur gemessen. Mit Hilfe der Messungen wurde das FE-Modell des gesamten Brustkorbes validiert. Die absolute Verschiebung aus den Messungen wurde mit der Simulation nicht erreicht, jedoch ist Richtung der Verschiebung der Brustkorbes vergleichbar mit denen aus der Messung. Der Fehler zwischen den simulierten und gemessenen Deformationen beträgt 40%. In der Simulation wurde innerhalb der Rippe eine maximale Spannung von 4 - 8MPa berechnet.Diskussion: Durch die biomechanischen Messungen sowie den Simulationen wurden viele Erkenntnisse über die Thorax Biomechanik gewonnen und die Spannungen in den Rippen konnten ausgewertet werden. Um jedoch die Konstruktion von Rippenimplantaten zu optimieren, müssen weitere alltägliche Belastungen wie Atmen, seitliche Bauchlage, Stöße auf den Thorax oder Husten simuliert werden.

Motivation: A resection of chest wall tumours leads to a loss of stability of the thoracic cage and a reconstruction of the chest wall is necessary. Large defects are treated with titanium plates. 84.6% of patients with these rib implants require a surgical revision due to an implant breakage. The construction of rib implants and their fixation is a substantial challenge and the rib implants must be designed for each patient individually. The knowledge of the biomechanical loading of a rib would be a major benefit in the development of rib implants. Thus, in this Master thesis, a FE-model of the thoracic cage is created to simulate the rib kinematics and to evaluate the stresses within the ribs. Material and Methods: In order to represent the biomechanics of the rib cage correctly by means of FEM, the stiffness and deformation behaviour of the entire thorax, the costal cartilage and the rib-vertebrae articulation were measured on three fresh human cadavers at the Medical University of Graz. A FE-model of the thoracic cage was created from CT data provided by the Medical University of Graz and with the obtained material parameters from the measurements. FE test models from the intervertebral disc, the rib-vertebrae articulation, and the costal cartilage were created to verify the modelling methods with the data from the measurements and from literature. Results: Within the bending measurements of the costal cartilage, the median Young’s modulus in the cranial/caudal direction (E = 23.4MPa) is greater than in ventral/dorsal direction (E =15.3MPa). The torsional stiffness of the cartilage is similar in dorsal-caudal and ventral-caudaldirection. A median shear modulus of 9.4MPa is determined. The deformation behaviour of thecartilage could not be correctly represented with the FE-model and the error between the simulatedand measured deformations in cranial direction is 42%. At the rib-vertebral articulation an averagedROM in the neutral zone of 15.5!±5.8! is measured. The tensile stiffness of the joint in longitudinal, sagittal and transverse direction is almost equal. Within the FE model, the deformation behaviour of the rib-vertebrae articulation, can also be represented. The force-displacement response of the entire thorax is measured with and without intercostal muscles. These measurements are used to validate the FE-model of the global thorax. Although the absolute displacement is not achieved with the simulation, the direction of the displacement is comparable to the measurements. The error between the simulated and measured deformations is 40%. Amaximumstress of about 4-8MPa is calculated within the ribs.Discussion: Through the combination of biomechanical tests and simulations it was possible to create an accurate Finite Element (FE)-model of the human thoracic cage to support the development of rib implants. However, in order to optimize the design of rib implants, further load cases such as breathing, lateral position of the thorax, impacts on the thorax or coughing have to be simulated.
