Kardiovaskuläre (CV) Erkrankungen sind hauptverantworlich für die Mortalität in den OECD Mitgliedsstaaten. Weltweit leiden rund 1 Milliarde Menschen an Bluthochdruck, dem wichtigsten unabhängigen Risikoindikator für CV Ereignisse (z. B. Herzinfarkt, Hirnschlag).<br />Unglücklicherweise können nur rund 50 Prozent der einschlägigen Todesfälle mit Bluthochdruck in Zusammenhang gebracht werden. Als dynamischer und vom Blutdruck unabhängiger Risikoindikator für CV-Erkrankungen gilt bspw. die arterielle Steifigkeit. Fokus dieser Arbeit war die Entwicklung von Algorithmen zur Messung von funktionellen kardiovaskulären Parametern, deren klinische Anwendung, sowie die Übertragung der gewonnenen Erkenntnisse in die Ausbildung junger Mediziner und Techniker. Neben der Entwicklung und Anwendung stellt die Validierung der vorgestellten Ansätze in Klinik und Lehre den zweiten Schwerpunkt der Arbeit dar. Technisch lag das Haupaugenmerk auf der Erweiterung einfach anzuwendender Standardmessmethoden, im Speziellen der oszillometrischen Blutdruckmessung. Mathematisch waren die Gleichungen zur Pulswellenmodellierung von Navier-Stokes in ihrer linearen Darstellung nach Womersley und die Windkesselgleichungen nach Frank zur Modellierung des ventrikulären Submodells dominant. Das pädagogische Konzept orientierte sich am Lerner-Centered-Design, einer Methode die unterschiedliche Ansätze vereint, wie etwa selbstständiges Arbeiten oder aktive Lernstrategien. Der Lehrinhalt wurde speziell auf die Zielgruppe abgestimmt und unterstützt komplexe Visualisierungen, sowie aktive Feedbackschleifen. Abschließend wurde sowohl die klinische Leistungsfähigkeit der Algorithmen, als auch der pädagogische Nutzen der Lernmodelle in Feldversuchen untersucht. In beiden Fällen konnte statistische Signifikanz im positiven Sinne belegt werden.<br />

Cardiovascular (CV) disease is the leading origin for morbidity and mortality in OECD countries. Worldwide about 1 billion people suffer from hypertension, which is the most important independent risk indicator for CV events. Unfortunately only 50% of CV death can be predicted by hypertension. Arterial stiffness within the Aorta turned out to be a significant predictor and may help to overcome existing diagnostic shortenings.<br />One aim of this work was the development of algorithms for measuring functional CV parameters, their clinical application and their use in medical education. The second goal of this work was the validation of the proposed approaches in clinical and educational practice.<br />Technical implementation was intented on affordable standard platforms like common oscillometric measurement systems using an occlusive blood pressure cuff. The governing equations were those developed by Navier and Stokes and their linear solution published by Womersley as well as the Windkessel equation introduced by Frank to model the ventricular sub system.<br />The principles behind the pedagogy included learner-centered design, which integrates self-exploration, active and meaningful learning strategies, motivational media, and visual teaching. The content was designed for end-users to visualize principles, to make predictions and to learn problem-solving skills with a hands-on approach, which allows to apply and test scientific methodologies and to see immediate outcomes from interactive manipulations. Finally field trials for both the clinical performance of the algorithms as well as the learning performance of simulation software were performed and showed significant positive statistical results.
