Trace elements in synovial fluids and placenta cryo-cuts with total-reflection XRF (TXRF)

Abstract

In dieser Arbeit wird das Vorkommen von bestimmten Metallen in menschlichen biologischen Proben (Synovialflüssigkeiten und Plazenta-Kryoschnitten) mittels Totalreflexions-Röntgenfluoreszenzanalyse (TXRF) untersucht. Synovialflüssigkeiten wurden untersucht, um die Konzentrationen der Elemente Ti, V, Cr, Co, Ni und Mo und einen Zusammenhang zum Abrieb von Metall-Implantaten zu bestimmen, welche zu unerwünschte lokalen Gewebereaktionen führen können (genannt „Metallose“). Plazenta-Kryoschnitte wurden sowohl aus normalen Geburten als auch aus Fällen von Frühgeburten und Fehlgeburten entnommen, um die Konzentrationen der Schwermetalle As, Cd, Hg und Pb zu bestimmen, die Störungen biologischer Prozesse während der Schwangerschaft verursachen können. Um zuverlässige Messungen zu gewährleisten, wurde eine geeignete Präparationsmethode für die Synovialflüssigkeiten erarbeitet. Anschließend wurde eine Reihe von Messungen mit Synovialflüssigkeiten durchgeführt. Proben aus der Kontrollgruppe zeigten vernachlässigbare Konzentrationen der untersuchten Elemente, während Proben aus der Metallose-Gruppe deutlich erhöhte Konzentrationen aufwiesen. Diese Resultate zeigen einen klaren Zusammenhang zwischen einen Implantatverschleiß und Metallosefällen. Zusätzlich zeigte ein Vergleich zwischen Synovialflüssigkeiten von Patienten mit denselben Implantatmodellen große Unterschiede in den Metallkonzentrationen, was darauf hindeutet, dass die Metallfreisetzung hauptsächlich durch Verschleißprozesse und nicht durch ein bestimmtes Implantatmodell selbst beeinflusst wird. Bei den Messungen der Plazenta-Kryoschnitte wurden kein Cd, As, Hg oder Pb nachgewiesen, was darauf hinweist, dass die Konzentrationen unterhalb der Nachweisgrenze der TXRF liegen.

In this thesis the presence of certain metals in human biological samples (synovial fluids and placenta cryo-cuts) is analysed using total-reflection X-ray fluorescence (TXRF). Synovial fluids were taken from patients to determine the concentrations of the elements Ti, V, Cr, Co, Ni and Mo and their relationship to the abrasion of metal implants, which can lead to undesirable local tissue reactions known as "metallosis". Placenta cryo-cuts were obtained from ordinary childbirths, as well as from cases of premature births and miscarriages to determine the concentrations of heavy metals As, Cd, Hg, and Pb, which are associated with disruptions of biological processes during pregnancy. To ensure reliable measurements, a suitable preparation method for the synovial fluids was developed. Subsequently, a series of measurements were performed using synovial fluid. Samples from the control group showed negligible concentrations of the elements under investigation, while samples from the metallosis group exhibited significantly elevated concentrations. These results show a clear association between implant wear and cases of metallosis. Comparisons between synovial fluids of patients with the same implant models showed large differences in metal concentrations, indicating that metal release is primarily influenced by wear processes rather than by the implant model itself. In the measurements of the placenta cryo-cuts no Cd, As, Hg or Pb were detected, indicating that the concentrations are below the detection limit of TXRF.
