Schwere, andauernde, teils unheilbare Erkrankungen stellen für betroffene Menschen einen drastischen Einschnitt in ihrem Leben dar. Nach der initialen ärztlichen Diagnose verläuft die Abfolge von teuren Krankenhausaufenthalten und Aufenthalten zu Hause in immer enger werdenden Zyklen. Die oftmals nicht optimale ärztliche Obsorge zu Hause in Kombination mit fortschreitendem Krankheitsstadium bedingen dies. Zusätzlich gilt es für die Patientinnen und Patienten eine Vielzahl neuer Herausforderungen zu meistern. Diese Tatsachen bergen ein großes Potential der Überforderung und können eine weitere Reduktion der Lebensqualität bedingen. Eine Intensivierung der ärztlichen ambulanten Versorgung scheint die logische Konsequenz, wobei eine Dauerversorgung vor Ort durch medizinisches Personal in Hinblick auf Umsetzbarkeit unrealistisch ist. Die Verbesserung der Versorgungssituation kann durch den Einsatz von Telemedizin erreicht werden. Im Zuge eines User Centered Design Prozesses wurden in Zusammenarbeit mit Ärzten und Patienten die Anforderungen, das Konzept und eine initiale prototypische Umsetzung eines patientenzentrierten Telemonitoring Systems zur Verbesserung der Kommunikation und Dokumentation im Bereich der mobilen Versorgung von Schwerkranken entwickelt. Während unser Prototyp einen ersten Einblick in eine mögliche Umsetzung der Dokumentation und des Monitoring aus Sicht des Patienten gibt, liefern die gesammelten Anforderungen einen Gesamtüberblick für ein entsprechendes System. Die in dieser Arbeit gewonnenen Erkenntnisse bilden eine Basis für künftige Entwicklungen auf dem Gebiet der telemedizinischen mobilen Betreuung, wobei die Kommunikation mittels neuer Medien zwischen Arzt und Patient von besonderer Bedeutung ist.

Incurable diseases represent a dramatic turning point in the lives of people affected. After the first medical diagnosis, the periods of consecutive time spent at home gradually decrease. The development of gradually deteriorating health condition is leading to more frequent, expensive stays in hospital. This is predominantly caused by inadequate medical care at home, often in combination with the progress of the disease. Furthermore, patients are challenged to cope with a variety of new challenges. Under such circumstances a reduction in quality of life is hardly avoidable. Intensifying of medical outpatient care seems to be a logical consequence. Yet, providing a permanent home care by medical personal is unrealistic in terms of feasibility and cost. However, improvement in medical care can be achieved through the use of telemedicine. In order to improve communication and documentation in the field of mobile care of seriously ill patients a user-centered design process was applied. In collaboration with physicians and patients the requirements, the concept and an initial prototype implementation of a patient-centered telemonitoring system was developed. Our prototype offers a first glimpse into a possible implementation of a patient's driven documentation and monitoring system. The gathered information further provides a good overview of requirements for corresponding systems. The results of this study are providing a cornerstone for future developments in the field of mobile telemedical care, where communication between doctor and patient via new media is of particular importance.
