Mit dieser Magisterarbeit werden sowohl Vergleiche und Analogien der einzelnen Standards im Datenaustausch medizinischer Dokumente kompakt (graphisch) vorgestellt, als auch in erster Linie eine Einsatzmöglichkeit jener Technik demonstriert. Dieser Praxisbezug wird durch eine Implementierung des Pflegeentlassungsbriefes hergestellt.<br />Bisher konnten in diesem medizinischen Teilbereich keine gemeinsamen Formate gefunden werden, wodurch zuerst die notwendigen Daten aus unterschiedlichen Pflegeentlassungsdokumenten gesammelt und ausgewertet werden mussten. Ausgehend von dieser Datensammlung konnte ein Musterbeispiel erstellt werden, der die Extensible Markup Language (XML) sowie die dazugehörende Extensible Markup Language Schema Definition (XSD) als Grundbasen benutzt. Bemühungen zum elektronischen Datenaustausch im medizinischen Bereich anhand eines XML Ansatzes gibt es bereits seit einigen Jahren. Bisher wurde jedoch meistens eine eigenständige Implementierung angewendet. Als Vorlagen hierfür kann man die frei zur Verfügung gestellten XML Dokumentationen des ON-Komitees betrachten. Aufgrund dessen, das solche Ansätze nur lokal bedingt anwendbar sind (für ONorm-Regeln bezieht sich dies österreichweit) wurde nach einer neuen Vorgehensweise gesucht, die auch internationale Bestrebungen mit einbezieht. Die aktuellste Clinical Document Architecture (CDA Version 2.0 - CDA V2) Struktur der Health Level 7 Organisation erwies sich dafür als besonders geeignet. Eine 5-stufige Vorgehensweise wurde gewählt: 1. Erhebung der Daten, 2.<br />Verallgemeinerung der Begriffe aus den gewonnenen Daten, 3. Erstellung eines ontologischen Modells, 4. Mapping des Modelles auf RIM (Reference Information Model) Objekte, 5. Extrahieren der aus RIM erstellten Objekte in ein CDA V2 konformes Dokument. Diese Magisterarbeit umfasst primär Punkt fünf mit Konzentration auf der Erstellung eines CDA Dokumentes.

The result of this master thesis is an XML ("Extensible Markup Language") and XSD ("Extensible Markup Language Schema Definition") prototype structure for a special electronic, medical document: the (nurse) care discharge document based from the Clinical Document Architecture 2.0 (CDA V2). The diploma analyses the international standards and compares them. The CDA (part of the "Health Level 7" - HL7 - framework) is an international approved standard. Therefore it was selected as the best solution for this work. The collection of data-fields from different discharge documents (of Austria) was also an important and extensive step. After that step, an ontological model was created, to afterwards extract this model into an XSD framework code.
