Abstract:
Currently electrohydraulic ESWT suffers from a lack of methods to determine the exact amount of generated pressure and its distribution inside the tissue due to stochastic variations. Single-point measurements can be used to assess the pressure at selected points, but they require the implantation of measurement devices and it is not possible to accurately evaluate the whole pressure field from them. In this thesis shock wave simulations are introduced as promising alternative for the determination of pressure fields in water baths and even after tissue interaction. First, an overview of shock waves and ESWT is given and the simulation program, which is based on the Matlab toolboxk-Wave, is described. Then the impact of different effective spark center positions and spark approximations on the pressure field is investigated to prove the necessity of accurate spark modeling in the simulations. Lastly, shock wave applications on a spinal cord are simulated to demonstrate the possibilities of realistic shock wave treatment simulations. Different application options are compared to identify the best qualified alternative for generating high pressure inside the tissue.
Momentan ist es aufgrund stochastischer Variationen nicht möglich, den während einer Stoßwellenbehandlung im Gewebe entstehenden Druck genau zu bestimmen. Zwar kann Druck an einzelnen Stellen gemessen werden, jedoch erfordern diese Messungen die Implantation von Sonden und selbst dann kann das gesamte Druckfeld nicht anhand einzelner Punktmessungen exakt berechnet werden. In dieser Diplomarbeit werden Stoßwellensimulationen als vielversprechende Alternative zur Bestimmung von Druckfeldern sowohl im Wasserbad als auch im Gewebe vorgestellt. Zu Beginn der Arbeit werden Grundlagen der Stoßwellentheorie und der extrakorporalen Stoßwellentherapie behandelt und das auf der Matlab toolbox k-Wave basierende Simulationsprogramm beschrieben. Weiters wird der Einfluss verschiedener Funkenpositionen und Funkenapproximationen auf das generierte Druckfeld untersucht, um die Notwendigkeit einer akkuraten Funkenmodellierung in den Simulationen zu verdeutlichen. Zum Schluss wird eine Stoßwellenbehandlung der Wirbelsäule simuliert, um die Möglichkeiten die sich durch realistische Simulationen von Stoßwellenbehandlungen ergeben zu demonstrieren. Verschiedene Anwendungsmethoden werden verglichen, um die für eine Stoßwellentherapie am besten Geeignetste zu bestimmen.
