Ziel dieser Arbeit war die Messung von standardized uptake values (engl., SUV) für Staging und Therapiekontrolle mit einem Gammakamera-Positronenemissionstomographen (GCPET) mit Röntgen-Schwächungskorrektur. Dies sollte am GCPET der Universitätsklinik für Nuklearmedizin der Universität Wien im AKH erreicht werden. Dazu war es notwendig, die prinzipielle Tauglichkeit des Gerätes für derartige Messungen zu analysieren, und eine entsprechende Meßmethode zu entwickeln.<br />Das untersuchte Gerät ist von der Type Millennium VG mit Hawkeye der Firma General Electrics Medical Systems (GEMS). Das Hawkeye ist ein Röntgencomputertomograph (RCT) mit niedriger Dosis.<br />Zunächst wurde die prinzipielle Tauglichkeit des Systems zur Detektion heißer Läsionen untersucht. Dies erfolgte für zwei Konfigurationen des GCPET, das zuerst mit einem 5/8 Zoll Kristall ausgestattet war, und dann auf einen 1 Zoll Kristall aufgerüstet wurde. Weiters wurden die Leistung des Röntgencomputertomographen (Hawkeye) und die Qualität der abgeleiteten Schwächungsbilder sowie der Koregistrierung zwischen GCPET und RCT überprüft. Für letztere wurde vom Autor ein eigenes Phantom ("Bricklet") entwickelt. Aufgrund des Systemaufbaus der Millennium VG hatte die Absolutkalibrierung mit Hilfe eines mitgemessenen Standards zu erfolgen.<br />Zu diesem Zweck wurde vom Autor ein weiteres Phantom (im Folgenden als "Referenzphantom" bezeichnet) entwickelt. Mithilfe dieses Phantom erfolgten für den ganzen klinisch zulässigen Meßbereich des GCPET Absolutmessungen. Weiters wurden dynamische Messungen durchgeführt.<br />Diese sind eine weitere, in dieser Arbeit entwickelte Neuerung, und können nur mit Geräten der untersuchten Type erfolgen, da diese über eine Slip-Ring Gantry verfügen.<br />Weiters wurden die Effekte möglicher geräte- oder patientenbedingter Fehlregistrierungen untersucht. Auch wurde eine erweiterte Methode zur täglichen Qualitätskontrolle (QC) entwickelt. Wo dies zweckmäßig erschien, wurden die Messungen am Vollringscanner der UKNM (ein Gerät der Type GEMS Advance) wiederholt.<br />Das Referenzphantom liefert genaue Werte für die Aktivitätskonzentrationen, aus denen SUVs berechnet werden können. Das Bricklet läßt die Bestimmung der Verschiebung zwischen Bildern des GCPET und RCT-Schwächungsbildern selbst für Werte unter der Schichtdicke des Hawkeye zu. Die neue QC-Methode erlaubt die Detektion von Störungen in der Koinzidenzdetektion, noch bevor diese die Bildgebung beeinflussen.<br />Die Kontrastauflösung des GCPET für Kontraste von 8 zu 1 ist hinreichend für den Nachweis von Läsionen mit 13 mm Durchmesser für das System mit 1 Zoll Kristallen, übertrifft also die Leistung von Systemen mit 5/8 Zoll Kristallen ganz wesentlich.<br />Die Daten der dynamischen Messungen zeigen, daß bei höherem Kontrast auch noch wesentlich kleinere Läsionen hinreichend genau gemessen werden können.<br />Obwohl das GCPET Millennium VG mit Hawkeye wie alle GCPET Systeme über eine wesentlich schlechtere Systemleistung verfügt als Vollringscanner, können bei einer Ausstattung mit 1 Zoll Kristall mit der entwickelten Methode für hinreichend große Läsionen Absolutmessungen der Aktivitätskonzentration vorgenommen werden. Die dabei erreichte Genauigkeit von besser als 10 Prozent erlaubt es, für klinische Anwendungen wie Staging oder Verlaufskontrolle SUVs zu bestimmen.<br />

The aim of this work was the measurement of standardized uptake values (SUV) for patient staging as well as therapy control with a gammacamera positron emission tomograpy system (GCPET) with attenuation correction derived from comeasured xray-computed tomography data (XCT).<br />This was to be achieved for the GCPET system at the Department of Nuclear Medicine (UKNM) of the University of Vienna at the Vienna General Hospital (AKH). For this goal it was necessary to investigate the feasibility of the measurement of absolute activity concentrations by means of this device and, if possible, develop a method to do so. The GCPET used in this work is a Millennium VG with Hawkeye by General Electrics Medical Systems (GEMS).<br />At first the system's capability for lesion detection was evaluated.<br />This was done for two configurations of the GCPET system which was upgraded from the initial thickness of the szintillation crystals of 5/8" to 1". Furthermore, the XCT performance and the attenuation maps derived thereof, as well as the quality of the coregistration were determined. For the latter task, a novel phantom ("bricklet") was developed by the author.<br />The system's configuration necessitated the absolute calibration to be performed by means of a comeasured standard activity. For this purpose, another phantom (termed "reference phantom") was developed during the course of this work. Measurements utilizing the reference phantom were performed at different countrates, covering the entire range of activity concentrations expected for clinical imaging. Dynamic measurements were performed as well. This represents another development of this thesis, only possible for the actual system which is equipped with a slip-ring gantry.<br />Possible effects of misregistration of emission and transmission data were evaluated for equipment-related effects and patient movement, and an advanced method for the daily quality control (QC) was developed as well. Where appropriate, measurements at the GCPET system were compared to those performed at a dedicated PET system (GEMS Advance), also situated at the UKNM. The reference phantom provided accurate absolute activity concentration data to be used for the calculation of SUV. The bricklet allows to determine offsets of emission and transmission data even below the axial resolution of the XCT. The novel QC method provides information that allows the recognition of problems in the coincidence detection mechanism even before they affect the imaging capabilities.<br />The system's lesion detection capabilities for activity concentrations of 8 to 1 are in the range of 13 mm for the 1" crystal, outperforming by far the previous system equipped with the 5/8" crystal. The data derived from dynamic studies show that, for higher contrast, even smaller lesions can be accurately imaged.<br />While the Millennium VG, such as any GCPET, fares worse in terms of lesion detectability than a dedicated PET system, the new method allows the measurement of absolute activity concentrations of lesions of sufficient size, if a 1" crystal is utilized. The accuracy achieved thereby, being better than 10%, allows the calculation of SUV for staging and therapy control.
